The importance of patient involvement is increasing in healthcare, and initiatives are constantly implemented to reach the ideal of involved and educated patients. This secondary analysis was initially embedded in a randomized controlled study where the aim was to gain insight into perceptions and experiences within a group of women undergoing fertility treatment through two focus group interviews. In this secondary analysis, we investigated how patient involvement was strived for in both clinical practice and research. During the analysis, it became (...) apparent that the women exercised and maintained a clear perspective on their hope for a child, Project Child, while the interviewer pursued a treatment perspective, Project Treatment. Despite different perspectives, the conversation during the interviews seemed effortless, and it became apparent how the interviewer and the participants were actually focusing partly on the same, but primarily on different issues but without addressing or acknowledging this. Knowledge and awareness of the difference in perspectives is important when healthcare professionals seek to involve patients both in clinical practice and in research. Patient involvement in both research and clinical practice has shown to be a challenge and entails that pathways are organized and decisions shared by healthcare professionals. (shrink)

Most respondents have made decisions that involve hastening the death of a patient, and almost all find it acceptable to do so. Such decisions are made more often, and considered ethically more acceptable, with the informed consent of the patient than without. But both non‐resuscitation decisions, and decisions to provide pain relief in doses that will shorten the patient's life, have been made (...) and found acceptable by at least 50% of the respondents, even when there is no informed consent. Furthermore, 12% have doubled morphine dosages with fixed intervals, thus providing doses substantially higher than that necessary to control pain, without the informed consent of the patient. Two per cent have helped in assisted suicide, and 5% have administered a lethal injection at the patient's request. Respectively 37% and 34% find these last two practices ethically acceptable. Amongst those that do not find them acceptable, the most important reasons to be opposed are, the doctrine of double effect, the doctrine of doing and allowing, and the view that human life is sacred. Amongst supporters, the most important reasons mentioned are, that the patient's right to self‐determination should be respected, the view that a patient should not be forced to suffer, and the view that the patient has a right to be helped to a dignified death. Many guidelines and commentators endorse the view that clinical research is ethically acceptable only when it has social value, in the sense of collecting data which might be used to improve health. A version of this social value requirement is included in the Declaration of Helsinki and the Nuremberg Code, and is codified in many national research regulations. At the same time, there have been no systematic analyses of why social value is an ethical requirement for clinical research. Recognizing this (...) gap in the literature, recent articles by Alan Wertheimer and David Resnik argue that the extant justifications for the social value requirement are unpersuasive. Both authors conclude, contrary to almost all current guidelines and regulations, that it can be acceptable across a broad range of cases to conduct clinical research which is known prospectively to have no social value. The present article assesses this conclusion by critically evaluating the ethical and policy considerations relevant to the claim that clinical research must have social value. This analysis supports the standard view that social value is an ethical requirement for the vast majority of clinical research studies and should be mandated by applicable guidelines and policies. Organizational ethics and institutional theories are extended by recovering Weberian and Pre-Weberian theorizing that emphasized the joining of ethics and institutional theories. Understanding how ethics and institutional systems influence each other can advance our understanding of the nature and causes of structural organizational ethics issues and help guide potential reforms. We consider the interplay of these elements during the recession of 2008–2009, highlighting how structural ethics problems may have to be addressed at the institutional levels and not solely the individual (...) or organizational levels. This paper explores how civic engagement as an important dimension of public engagement in higher education has been slow to develop in the UK, despite an important history dating from the ‘civic universities' in the ninetheenth century. I specifically consider the development of ' service learning ' as an important way in which the values and practices of democratic citizenship can be embedded in the curriculum of higher education. Finally, I examine how the decline of the ideal of ‘public service' (...) in the UK provides some significant barriers to the re-development of the civic university. (shrink)

A widespread view among contemporary philosophers and scientists is that the soul is a mystification. For Marilynne Robinson, American essayist and novelist, the crux of the matter is not the existence of the soul in itself, since this cannot be settled by debate. Rather, she challenges the sort of evidence that her opponents—mostly basing themselves on the work of neuroscientists, and evolutionary biologists—deem to be decisive in determining the question. The soul, she claims, does not appear at the level of (...) our genes and neurons. Rather it is encountered in the many works of art and reflection that human beings have produced from the earliest times. This paper will focus on one such document, Robinson’s novel Gilead, in which she proposes a vision of the soul closely allied to the notion of blessing. Blessing, in turn, is inseparable from metaphor, pointing us to mystery, an elusive reality whose presence we experience only intermittently, although it is always there. Although Robinson’s several collections of essays provide needed context for the view of the soul displayed in the novel, it is our claim that it is the novel that truly turns the tables in the debate, inviting the reader to affirm or deny the soul’s reality not on the basis of the pronouncement of experts but on the basis of the way a given language aligns with experience. The internalization that such a process requires reveals the soul in action. This paper is thus a reading of Robinson’s writings on the soul. (shrink)

This essay examines the contrast between two conceptions of the universal, one represented by the modern State and the other by the Jewish people. In order to do so, it returns to the collection of essays on Judaism Levinas wrote in the approximately two decades after the Second World War, Difficult Freedom . Its aim is to focus specifically on the political dimension within this collection and then to step back and reflect on how his way of speaking of the (...) political appears to us a full generation later. As is well known, Levinas's approach to the political has a way of escaping that realm, while nonetheless remaining relevant to it. This is what we shall try to capture and to evaluate. (shrink)

In one of his last essays, "Clio--Dialogue de l'histoire et de l'âme païenne," Charles Péguy meditates at length on the human being's position in time, what he sometimes calls the secret of the man of forty. It is an inescapable secret to which all people are privy, provided they live to the requisite age. Once one knows the secret, it reshapes one's relationship to others and, as a result, what one notices about them, the evidence itself. Péguy provides several examples (...) of what historiography might look like if the historian proceeded from the secret of the man of forty. It amounts to embodying a certain solidarity with those one is investigating, based not on party affiliation, religion, nationality, race or gender, but on a common defeat in time and yet a sort of triumph within that defeat. Péguy accuses the positivists of pretending they don't know the secret of the man of forty. That is, he accuses them of willfully ignoring their own frailty. As a result, they fail to establish a common bond with the people they are studying. For Péguy, the elaboration of that common bond is prerequisite to all historical investigation. There are, it is argued, conceptually and empirically adequate naturalistic explanations of religion that explain religion without explaining it away and without leaving out anything needed fully to comprehend religious phenomena. Moreover, naturalistic explanations arc sometimes also critiques of religion. This article concerns itself with a subspecies of such explanations through articulating and defending some naturalistic criticisms of the truth-claims of religion. The rationale is displayed for naturalistic thinkers going from truth-claim analyses to functional analyses and the central naturalistic explanations (...) of the roles and functions of religion are distinguished and related. It is shown how these analyses dovetail and, particularly when supplemented by an error-theory of religious belief, constitute a comprehensive and adequate explanation of religion. (shrink)

Walter Fisher's narrative paradigm has sparked intense debate about the role of narratives in rhetorical scholarship. The theory has subsequently been followed up by numerous criticisms and revisions. This article argues that especially the latter can benefit from a complementary phenomenological perspective that Fisher himself placed within the original paradigm. Through the Heideggerian concepts of building and dwelling, rhetoric within the narrative paradigm may be seen as a primary means for engaging with both individual and communal goals and exigencies, providing (...) a more nuanced account for how people care for and act on the narratives in their lives. The aim of this article is to analyse entrepreneurship from an action research perspective. What is entrepreneurship about? Which are the fundamental capabilities and processes of entrepreneurship? To answer these questions the article includes a case study of a Danish entrepreneur and his networks. Finally, the article discusses how more long term action research methods could be integrated into the entrepreneurial processes and the possible impacts of such an implementation.

The aim of this study was to explore the ethical challenges in home mechanical ventilation based on a secondary analysis of qualitative empirical data. The data included perceptions of healthcare professionals in hospitals and community health services and family members of children and adults using home mechanical ventilation. The findings show that a number of ethical challenges, or dilemmas, arise at all levels in the course of treatment: deciding who should be offered home mechanical ventilation, respect for patient and family (...) wishes, quality of life, dignity and equal access to home mechanical ventilation. Other challenges were the impacts home mechanical ventilation had on the patient, the family, the healthcare services and the allocation of resources. A better and broader understanding of these issues is crucial in order to improve the quality of care for both patient and family and assist healthcare professionals involved in home mechanical ventilation to make decisions for the good of the patient and his or her family. This interdisciplinary volume addresses the tension between illusion and appearance. The starting point is three differentiated forms of "Schein" in German: as a ray of light itself, as in "shine," or appearance as becoming visible or, finally, as an illusion – seeming to be so. The contributions analyze under which cultural-historical conditions and within which concrete forms aesthetic configurations are perceived and evaluated.

Tool making has been proposed as a key force in driving the complexity of human material culture. The ontogeny of tool‐related behaviors hinges on social, representational, and creative factors. In this study, we test the associations between these factors in development across two different cultures. Results of Study 1 with 5‐to‐6‐year‐old Turkish children in dyadic or individual settings show that tool making is facilitated by social interaction, hierarchical representation, and creative abilities. Results of a second explorative study comparing the Turkish (...) sample with a sample of 5‐to‐6‐year‐old children in New Zealand suggest that tool innovation might be affected by culture, and that the role of cognitive and creative factors diminishes through social interaction in tool making. The average annual profits before fees of the $10 billion plus Renaissance Technologies’ hybrid Medallion “Leveraged, High Frequency, Artificial Intelligence ” trading hedge fund between 1988 and 2019 were about 66 percent. Total trading profits during this period were over $100 billion. The fund has never had a losing year. The fund is not open to the general public. First, distinctions among, in more or less historical order, the traditional market-maker trading model, the hedge fund trading model, the artificial intelligence (...) trading model, and the hybrid LHFAI trading model are discussed. Second, the micro components of the LHFAI trading model are explained in the context of Renaissance Technologies’ Medallion Fund. Third, key positive contributions of the model with respect to profitability, low annual volatility, market liquidity, and intellectual property development; negative ethical issues concerning exclusive access, tax fairness, financial transparency, shared responsibility for losses and systemic risk, and short vs. long-term capital allocation are discussed. Potential reforms that retain the positives, reduce the negatives, and that could positively transform the model are discussed. Fourth, potential impacts that the potential reforms might have on the macro LHFAI form of finance capitalism and the larger finance capitalism political-economic system are considered. The cultural logic contained within Emile Durkheim's work, specifically categories he puts forth in Suicide, creates the ground for Horrible Workers. This book is constructed to allow its readers to study the cases of Max Stirner, Arthur Rimbaud, Robert Johnson, and the Charles Manson Circle independently of one another or in a comparative fashion. Each case demonstrates in what ways particular social experiences lead to what have been perceived as unique forms of cultural expression.

This article investigates different acts of political protests currently floating from unemployed citizens who are being affected by recent retrenchment policy reforms. Whereas most of the existing literature tends to portray political protest as either collective and public or individual and private, this article attempts instead to shed light on the plurality of normative resources activated by the unemployed in a highly critical situation. Thereby the analysis moves between the collective and the individual as well as between the public and (...) the private. Using the theoretical framework developed by Laurent Thévenot and Luc Boltanski in their joint work on justification, the article analyses a specific case, namely unemployed Danish recipients of social assistance who are affected by a new policy initiative meaning that their income has been lowered. Drawing on newspaper articles and qualitative in-depth interviews with affected citizens, the analysis unfolds and theorizes upon three very different forms of protesting: a civic, an industrial and a domestic form of resistance. This article argues that personal meaning should be considered important when addressing issues of learning. It is claimed that meaningful learning is not primarily intra-psychological, as suggested by humanistic psychologists and parts of cognitive psychology, but is an integrated part of the person’s participation in various social practices. Inspired by critical psychology and situated learning, it is suggested that in order to comprehend what people in everyday life experience as meaningful, we have to understand the concerns subjects pursue across different (...) contextual settings and the kind of conduct of everyday life they try to realise. A case example from an ongoing research project about how baker apprentices learn their trade is outlined in order to exemplify some of the theoretical considerations. Two baker apprentices, Peter and Charlotte, are presented to illustrate how they orientate their learning activities in the bakeries according to their future participation in the baking trade and in relation to the conduct of everyday life they wish to pursue.
